In the USP classification scheme, PPIs constitute one drug class in the larger category of gastrointestinal agents; as such, the statutory requirement requires that plans cover at least two PPIs. For the set of drugs in this class, there seems to be little evidence for the clinical superiority of one product over another. Thus, it is a class where plans have a fairly high incentive to select a preferred product and where beneficiaries should be able to switch products with relatively little difficulty. Beneficiaries with a preference or a current prescription ; for one particular product will see large differences across these 14 plans. A Enxium user will save 8 by picking Medco over Wellcare Signature, while a Protonix user will save 4 by picking Aetna Essentials over Medco and another by picking Cigna Value. There are many ways to go about treating chronic cases of acid reflux. Many people advocate the use of natural home remedies to treat acid reflux. Summary: Infection with Histoplasma capsulatum occurs commonly in areas in the Midwestern United States and Central America, but symptomatic disease requiring medical care is manifest in very few patients. The extent of disease depends on the number of conidia inhaled and the function of the host's cellular immune system. Pulmonary infection is the primary manifestation of histoplasmosis, varying from mild pneumonitis to severe acute respiratory distress syndrome. In those with emphysema, a chronic progressive form of histoplasmosis can ensue. Dissemination of H. capsulatum within macrophages is common and becomes symptomatic primarily in patients with defects in cellular immunity. The spectrum of disseminated infection includes acute, severe, life-threatening sepsis and chronic, slowly progressive infection. Diagnostic accuracy has improved greatly with the use of an assay for Histoplasma antigen in the urine; serology remains useful for certain forms of histoplasmosis, and culture is the ultimate confirming diagnostic test. Classically, histoplasmosis has been treated with long courses of amphotericin B. Today, amphotericin B is rarely used except for severe infection and then only for a few weeks, followed by azole therapy. Itraconazole is the azole of choice following initial amphotericin B treatment and for primary treatment of mild to moderate histoplasmosis.

On 08 Doral Dental, the dental provider, received a request for prior authorization for orthodontic treatment, specifically braces, for the Appellant. 3. This request was denied on 08 28 due to a lack of medical necessity. 4. The Appellant's dentist notes that tooth #13 is in a cross-bite and that tooth #18 is missing. He also notes that the Appellant's teeth show a marked asymmetry. 5. Buckeye Health, through Doral Dental, determined the appellant did not have a deep impinging over bite, a true anterior open bite, a large anterior-posterior discrepancy, an anterior cross bite, a posterior transverse discrepancy, significant posterior open bites, or impacted canines that will not erupt without intervention. 6. Buckeye Health determined there was not a situation of severe handicapping, and determined that braces were not medically necessary. 7. Buckeye Health based their decision on panoramic x-rays and models submitted with the prior authorization request. 8. The Appellant had previously been approved for and taking the medication Nexium 40mg. 9. The Appellant took Prilosec OTC as required by the prior authorization process both once and twice a day. 10. The Appellant's physician reports that symptoms were not adequately controlled while taking Prilosec OTC See Exhibit 2 ; . 11. The Appellant's physician reports significant improvement of symptoms while taking Nexium and believes that it is in his patient's best interest to take Nexium for control of GERD symptoms and allopurinol.
During April Hanmi Pharm. is scheduled to submit an investigational new drug IND ; application for Esomezol an incrementally modified drug or IMD ; to the US Food & Drug Administration. Esomezol is a drug where strontium sulfate is used instead of magnesium sulfate. If AstraZeneca files a patent lawsuit against Hanmi Pharm., and as a result a 30-month grace period is introduced, the company will be able to release Esomezol after Oct 2010. Hanmi Pharm. is currently negotiating with a few pharmaceutical companies for a license. If Nexium's patent rights are valid as agreed to by both AstraZeneca and Ranbaxy Laboratories, Teva IVAX and Dr. Reddy will not be able to release generic versions. Instead, Esomezol, which Hanmi Pharm. claims does not infringe upon Nexium's patent, but instead bypasses patent law, will become the first generic version of the drug. In this sense, the best scenario for Hanmi Pharm. would have been if AstraZeneca won its lawsuit against Ranbaxy Laboratories. Although the two parties instead reached a compromise, it is still positive that Ranbaxy has agreed not to release its generic version of Nexium until 2014. Given that Nexium is a blockbuster drug that generated total global sales of .2bn in 2007 alone, Hanmi Pharm. will be able to produce an enormous profit if it releases the first generic drug and preoccupies the market. However, if Teva IVAX and Dr. Reddy win their lawsuit against AstraZeneca, or if an authorized generic version of Nexium is released before Esomezol, Hanmi Pharm. will lose the opportunity to preoccupy the market.
