Cardiovascular Safety In the PROspective pioglitAzone Clinical Trial In macroVascular Events PROactive ; , 5238 patients with type 2 diabetes and a prior history of macrovascular disease were treated with pioglitazone or placebo. The primary endpoint was time to first occurrence of any event in a cardiovascular composite endpoint. Although there was no statistically significant difference between pioglitazone and placebo for the 3year incidence of a first event within this composite endpoint, there was no increase in mortality or in total macrovascular events with pioglitazone. The main secondary endpoint was time to first occurrence of any event in the composite of all cause mortality, nonfatal MI, & stroke. Pioglitazone demonstrated a significant risk reduction of 16% HR 0.84; P 0.027 ; vs. placebo. A pre-specified subgroup analysis of PROactive patients with previous MI demonstrated a significant risk reduction of 28% HR 0.72; P 0.045 ; in time to fatal nonfatal MI with pioglitazone compared to placebo. In PROactive, the percentage of patients who had an event of serious heart failure was higher for patients treated with pioglitazone 5.7%, n 149 ; than for patients treated with placebo 4.1%, n 108 ; , however heart failure mortality rates did not differ between groups. Robert Misbin, who was the primary review on Avandia when it was approved and is still the primary reviewer assigned to Actos, sees the outcome as rational. As a physician, Misbin says, he would not prescribe Avandia given the strong indication that Actos is the safer choice. But as a regulator, there should be a higher hurdle before the agency decides that no physician can ever prescribe Avandia. Even with those explanations, the two votes are complicated to reconcile. As a result, the meeting did nothing to quell public concern about FDA's handling of the drug. And now the agency has to make up its mind what to do with Avandia.

The Aviation Environment In terms of vision, the environment of pilots is characterised by the complexity of visual information used by the pilot; uncertainty about what is being seen; and the speed at which visual cues enter the visual field, move across it and disappear. It takes time when a pilot's brain uses data to form a mental `model' of the situation, and reaches a decision. If the visual environment is too complex because of dense air traffic, flying close to the ground, or degradation of visual information by distance, poor light or haze ; the visual functions may be too slow for the speed of the aircraft. For instance, a retinal image will `smear' if the object is moving very fast or is close to the viewer, as in low level flight. Agricultural aircraft often fly close to the ground in terrain filled with obstacles such as power lines and cables. b ; Operational Factors Affecting Vision Environmental factors will sometimes interfere with normal visual function. These include glare, flicker, low light, poor light or colour contrasts between viewed objects and their background. The visual performance of the pilot may also be reduced by fatigue or hypoxia.
